How to Get an
Application |

MEMBERSHIP:
Fast and EasyBecoming a member of the AccessHealth Program
does not have a cost to the qualified persons who find
themselves without medical insurance. The program is a
non-profit service provided by health care organizations,
government agencies and the community.
Here are the simple membership
requirements:

As a resident of Nevada you or members
of your family have no medical insurance and do not
qualify for Medicare, Medicaid or Nevada Check-Up. If
you do qualify for one of the public insurance programs, we
can assist you in the application process.
You
or members of your family have a job so that you can pay for
the reduced medical fees for the health care you need.
The services provided are not free, but will offer
care at a greatly reduced cost. Those services must be
paid by the member to the provider at the time of service.
Note: For those who are part of one of our special
programs such as Community Health Center patients needing
specialty care or those in the Ryan White program - the
employment requirement may be waived.

Your family's income fits in the chart
provided. Just look at how many people are in your
household and compare the total annual income in the chart
below. If that amount is between the "from" - "to"
amounts shown in the chart - you qualify to become a member!

Fill out the simple application. You
will then be contacted by the enrollment staff at
AccessHealth. If you do not already have a primary
care doctor, you will select one from our network! It will
be great to have a primary care home that you can afford.
No more sitting through the long wait at the local emergency
rooms for the primary care you need.
How It Works:
Affordable Quality Care
Once you are a member in
AccessHealth and are assigned a primary care home you are
on the road to leading a healthier life.
AccessHealth gives you an I.D.
card that identifies to the provider that you are to receive the
negotiated fees for AccessHealth members. Providers expect
payment at time of service for you to receive the full benefits
of your membership.
Your primary care physician will refer you to specialty care or
diagnostic tests if necessary. The AccessHealth staff will
assist in coordinating your appointments.

Jorge works
hard repairing electrical engines. He cannot afford
health insurance for his wife and three children.

Today
Jorge and his family can afford health care services,
thanks to their AccessHealth membership. The children
are now part of a free immunization program and
pediatric physicals offered to the community twice a
month. They all have primary care, medicines, dental
services and specialty services at a price they can
afford. Jorge says that being members of AccessHealth
links his family with safe and affordable medical
services. This gives him the peace of mind that he
needed. He wants to thank all the people at
AccessHealth for their wonderful work! |
