What is Nevada Check Up?

Nevada Check Up is the State of Nevada’s Children’s Health Insurance Program. It provides low-cost, comprehensive health care coverage to low income, uninsured children 0 through 18 years of age who are not covered by private insurance or Medicaid.

How do we apply?

The process is easy!

The head-of-household completes the application and mails it to the Nevada Check Up office:

Proof of all household income must be included with your application.

For wages or salary from jobs, enclose copies of your 2 most recent pay stubs.

If you are self-employed, enclose your most recent tax return (IRS Form 1040) along with Schedules C, E, or F for all businesses you own.

If you receive Social Security, Child Support, Disability, or any other form of unearned income, enclose a copy of the award letter, court order, a bank statement showing direct deposits, a copy of the check, or any other proof you may have that shows the income source, the amount, and how often you receive the money. 

Your Nevada Check Up application will be reviewed within 45 days and you will be notified by letter whether you are approved or denied for coverage.

Applications for Nevada Check Up are available throughout the state at locations including: Family Resource Centers, Schools, Boys & Girls Clubs, Welfare offices.
